The title of the application should describe the proposed new or changed listing. Provide an application title that is meaningful for you/your organisation when drafting it in the HPP.
Is this a resubmission of a previous application?
If you have applied to list on the Prescribed List for the same product previously, you will need to provide the previous application number, if known. If you are resubmitted within the HPP (from the Applications page) then this will be pre-filled for you.
Are you applying to Part A, Part B or Part C of the Prescribed List?
If you select Part A of the Prescribed List, you will be asked:
What is the pathway for this application?
When selecting the pathway for your application, please refer to the Prescribed List Guide. You can select from:
- Tier 1 – Departmental assessment pathway
- Tier 2 - Clinical HTA pathway
- Tier 3 - Full HTA pathway
Applications falling into the Tier 3 pathway will either be novel devices, or co-dependent applications dependent on a new or changed MBS Item. Tier 3 applications require applicants to first apply to MSAC.
If Tier 3 is selected, you will be asked:

Does your product rely on the listing of a new Medicare Benefits Schedule (MBS) item?
This question will determine if the application is co-dependent on an MSAC application.

If you select ‘Yes’ you will be required to search for and select the relevant lodged MSAC application.
If you select ‘No’, the following text will be displayed:
You will be prevented from lodging this Tier 3 application until you have lodged your MSAC application. Lodge your MSAC application from the Applications page.

A new product or product system on the Prescribed List
Submitting an application for a new product on the Prescribed List will allow you to propose a new Billing code to be listed on the Prescribed List.
Applications may be commenced in parallel to the ARTG Inclusion process, however, the product will not be approved for inclusion on the Prescribed List until approval has been granted and an ARTG ID has been allocated.
An amendment to an existing Billing code(s) on the Prescribed List
Submitting an application for an amendment to an existing item on the Prescribed List will allow you to modify the attributes of an existing Billing code or codes.
Changes may include, but are not limited to: catalogue numbers, grouping, product name/description/size, ARTG details or benefit.
If you select that your application is an amendment, you will be asked:
Provide a summary of the proposed amendment
Provide a summary of what you are proposing to amend in the existing billing code(s).
An expansion of a single product to multiple products on the Prescribed List (only available for selection for Tier 1 and Tier 2 applications)
Submitting an expansion application to the Prescribed List will allow you to take a Billing code that already exists on the Prescribed List and split it up into two or more new Billing codes.
You will not be able to introduce new catalogue items into the resultant Billing codes, nor modify the attributes of existing catalogue items. Duplicated catalogue items will need to be justified on the Product details page.
If you select expansion you will be asked:
What is the main reason for the expansion?
Select the most appropriate reason for the expansion application.

A compression of multiple products into a single product on the Prescribed List (only available for selection for Tier 1 and Tier 2 applications)
Submitting a compression application to the Prescribed List will allow you to take multiple existing Billing codes and merge them into a single, new Billing code.
You will not be able to introduce new catalogue items into the resultant Billing codes, nor modify the attributes of existing catalogue items. Duplicated catalogue items will need to be justified on the Product details page.
