Are you the sponsor, or are you an agent acting on behalf of the sponsor?
The sponsor is recognised as a member of the organisation responsible for the medical device or human tissue product.
An agent is an external person to the organisation (for example, a consultant) contracted to apply on behalf of the sponsoring organisation.
If you are an agent completing this application, all correspondence will also be sent to you, along with both primary and secondary contacts. If your agent role finishes, you must inform the Department as soon as possible (so the Department can remove you from its email distribution list for that application).

One primary contact must be added. This is to enable the Department to address key correspondence to one individual, where applicable.
Notifications for correspondence from the Department sent through the HPP will be sent to both primary and secondary applicant contacts.
